29 CFR 1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: At a job site located at 3000 Conway Drive in Raymore, Missouri: Employees were exposed to struck by/caught in injuries. Employees were working in a trench with no safe means of egress. The trench employees were working in was 4 feet deep and 30 feet in length with no stairway, ramp or ladder for egress.

29 CFR 1926.652(b)(2):Maximum allowable slopes, and allowable configurations for sloping and benching systems, was not determined in accordance with the conditions and requirements set forth in Appendices A and B to this subpart: At the construction site located at 3000 Conway Drive in Raymore , Missouri: Employees were exposed to struck-by and crushed-by hazards. The employer did not ensure that benching system that employees were using while working in trench was in accordance with the requirements. The bottom bench exceeded 4 feet in height and the maximum allowable slope exceeded 1:1.
