An employee was exposed to struck by hazards from rocks and other debris by not wearing personal protective equipment in the form of a hard hat while working in an unprotected trench approximately 15 feet in depth.

A stairway, ladder, ramp or other safe means of egress was not located in trench excavations more than 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. At a residential jobsite located at 4610 Forrest, Kansas City, Missouri- An employee was exposed to engulfment hazards in that there was no safe means of egress located in the approximately 16 x17 foot trench, approximately 15 feet at the deepest part, (north end) via stairway, ladder or ramp.
