29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) At the site, on or about 4/13/2022, employees working on a balcony on the third floor of Building #1, approximately 20 ft above ground level, were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems, exposing them to fall hazards.

29 CFR 1926.502(d)(21):Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: (a) At the site, on or about 4/13/2022, the personal fall arrest system used by employees to protect themselves from fall hazards was not removed from use despite having rust and deterioration on the metal components, exposing employees using it to fall hazards.
