Superior Rehabilitation, Center LLC. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1904.4(a), which was contained in inspection number 1504260, citation number 2, item 1 and was affirmed as a final order on June 4, 2021, with respect to a workplace located at 1800 New York Avenue Superior, WI 54880.
