Perkins in Stillwater, OK 73034, an employee was observed working in the basket of a bucket truck without the lanyard attached to the basket, exposing the employee to the hazard of falls.

Perkins in Stillwater, OK and times thereafter, injury and illness logs were not provided upon request.
