The employer had not conducted a hazard assessment and created a written certificate.  Employees were exposed to eye burns and blindness.

29 CFR  1910.303(b)(2):  Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  Facility:  On and/or before April 19, 2022, small appliances were plugged into relocatable power taps and not used according to listing and labeling instructions. Employee(s) were exposed to potential electrical shock and/or fire hazard(s).  In the following location(s): a) WAAS Room 238: Employee desk space: Keurig Coffeemaker  b) NEU: Employee desk space:  Keurig Coffeemaker c) First floor administrative Office:  Employee desk space:  Keurig Coffeemaker  The Federal Aviation Association was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.303(b)(2) which was contained in OSHA inspection number 1542915, citation number 1, item number 1, and was affirmed as a final order on August 31, 2021, with respect to a workplace located at 927 Flight Path Avenue, Las Vegas, NV, 89119.

29 CFR  1910.1200(g)(1):  Employers did not have a safety data sheet in the workplace for each hazardous chemical which they use   Facility: Maintenance:  Employees used floor repair products such as Quarry's Quikcrete water stop cement that contain Portland Cement.  The employer did not ensure a safety data sheet was located onsite and available to employees. The employees were exposed to dermal burns.

29 CFR  1910.1200(h)(1):Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  Facility: Maintenance:  Employees used floor repair products such as Quarry's Quikcrete water stop cement that contains Portland Cement.  The employer did provide and ensure effective training was conducted specific to safety and health hazards associate with the product. The employees were exposed to dermal burns.
