29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  The employer failed to protect employees from fall hazards.  This was most recently documented on or about April 19th, 2022, in Gretna, Nebraska 68028.  An extension ladder was used for access and egress to the upper level of the residential structure and it did not extend at least 3 feet above the surface being accessed. Also, the ladder was also not secured at its top to a rigid support.

29 CFR  1926.405(g)(2)(iv):Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.  The employer failed to protect employees from electrical hazards.  This was most recently documented on or about April 19, 2022, in Gretna, Nebraska.  Employees were using flexible cords that had exposed energized conductors.  Urbic Construction was previously cited for 1926.405(g)(2)(iv) a violation of this Occupational Safety and Health Standard , which was contained in OSHA inspection number 1573266, citation number 1, item number 2,and was affirmed as a final order on March  10, 2022, with respect to a workplace located at 11516 South 190th Street, GRETNA, NE 68028.

29 CFR  1926.416(e)(1):Worn or frayed electric cords or cables shall not be used.  The employer failed to ensure that all employees are protected from electrical shock hazards while utilizing damaged flexible electrical extension cords.  This was most recently documented on or about April 19, 2022 in Gretna, Nebraska. Employees were using damaged flexible electric cords with exposed internal conductors to supply power to equipment at the worksite.  Urbic Construction was previously cited for 1926.416(e)(1)  a violation of this Occupational Safety and Health Standard , which was contained in OSHA inspection number 1565946, citation number 1, item number 2,and was affirmed as a final order on January  19th, 2022, with respect to a workplace located at 11508 S 190th St, GRETNA, NE 68028.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  The employer failed to protect employees from fall hazards when conducting framing operations on a residential structure. This was most recently documented on or about April 19, 2022 at a residential construction jobsite located in Gretna, Nebraska. The exposed employees were standing on the finished OSB/trusses of the residential structure at heights over 6 feet and not utilizing any form of fall protection.  Urbic Construction was previously cited for 1926.501(b)(13)  a violation of this Occupational Safety and Health Standard , which was contained in OSHA inspection number 1573266, citation number 1, item number 4,and was affirmed as a final order on March  10, 2022, with respect to a workplace located at 11516 South 190th Street, GRETNA, NE 68028.
