29 CFR 1910.1025(d)(1)(ii):Full shift (for at least seven -7 continuous hours) personal samples for lead were not collected including at least one sample for each shift for each job classification in each work area: In the facility located at 100 Springdale Rd, Westfield, MA 01085: On or about April 12, 2022, laborers were assigned to work where lead containing dust/sludge is present including the manual removal of sludge from range traps and cleaning of lead containing dusts from work areas and the employer did not collect lead personal exposure samples for at least seven 7 continuous hours, for each job classification, in each work area.

29 CFR 1910.1025(d)(6)(iii): Where the initial monitoring revealed that employee exposure to lead was above the permissible exposure limit, monitoring was not repeated at least quarterly and continued at the required frequency until at least two consecutive measurements taken at least 7 days apart were below the PEL and at or above the action level: In the facility located at 100 Springdale Rd. Westfield, MA 01085: On or about May 5, 2022, and at times thereafter personal air monitoring for Lead revealed that an employee, performing Trap House Cleaning Operations, including the manual removal of sludge from range traps, cleaning of lead containing dusts from bullet traps and performing lead decontamination tasks, was exposed to an 8-hour time-weighted average airborne Lead level of 71 ?g/m3 lead which is 1.42 times the 50 ?g/m3 permissible exposure limit and monitoring was not repeated at least quarterly until at least two consecutive measurements taken at least 7 days apart were below the PEL.

29 CFR 1910.134(m)(2)(i): The employer shall establish a record of the qualitative and quantitative fit tests administered to an employee including: (A) The name or identification of the employee tested; (B) Type of fit test performed; (C) Specific make, model, style, and size of respirator tested; (D) Date of test; and (E) The pass/fail results for QLFTs or the fit factor and strip chart recording or other recording of the test results for QNFTs. In the facility located at 100 Springdale Road Westfield, MA: On or about 04/12/2022 employees, performing Lead removal from firing range traps, were required to wear half-face negative pressure respirators which required annual fit testing and the employer's fit test records lacked information on the type of fit test performed and the specific make, model, style and size of respirator tested.
