29 CFR 1910.216(c)(1): Safety tripod, cable, or wire center cord shall be provided across each pair of in-running rolls extending the length of the face of the rolls. It shall be readily accessible and operate whether pushed or pulled. The safety tripping devices shall be located within reach of the operator and the bite. a. Lamination Department - On or about April 11, 2022, a temporary employee of Employbridge dba. ProLogistix working at Herculite in performing lamination winding on the Laminator 1, when they were pulled into an unguarded calendar, which resulted in a hospitalization. b. Lamination Department - On or about April 20, 2022, employees were performing lamination winding on Laminator 1 calendar, employees were exposed to a caught in hazard. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
