29 CFR 1910.147(c)(8): Lockout or tagout was not performed only by the authorized employees who are performing the servicing or maintenance: a) HOMAG Ambition 2270 Jubilee, banding machine S/N 0-200-10-2804; on April 18, 2022, the CSHO observed an employee exposed to an amputation hazard in that the employee was performing maintenance while working under the lock of the host employer the employer did not ensure that employees apply their personal lock.

29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: a) HOMAG Ambition 2270 Jubilee, banding machine S/N 0-200-10-2804; on April 18, 2022, the outside servicing employer did not inform the onsite employer of their energy control procedures and did not enforce the use of the onsite employer's energy control program exposing the employee to amputation hazards.
