29 CFR 1910.147(c)(8): Lockout or tagout was not performed only by the authorized employees who are performing the servicing or maintenance: (a) At the Cabinet by Design, located at 2883 Pleasant Hill Rd, Duluth, Georgia at the Edge Banding machine S/N: 0-200-09-8990: on or about May 4, 2022, and at times prior, employees were exposed to an amputation hazard, while performing services and maintenance on the Edge Banding Machine, without the use of their own personal lockout device.
