29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: At a worksite located at 2950 Summit St, Kansas City, MO 64108: Employees were exposed to struck-by hazards. This was most recently demonstrated on or about April 8, 2022, when employees were permitted to operate pneumatic nail guns to install wooden trusses without wearing impact-resistant eye protection.

29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems or personal fall arrest systems: At a worksite located at 2950 Summit St, Kansas City, MO 64108: Workers were exposed to fall hazards. This was demonstrated on or about April 8, 2022, when workers were working above 25 feet placing trusses on a commercial building. Workers were without a tethering device, personal fall protection, or fall protection systems.
