29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At the jobsite located at 19650 W. 201st St. Spring Hill, Kansas, employees engaged in residential framing activities were exposed to fall hazards in the following instances: a) On the southeast corner of the partially framed residential structure, employees utilized Werner 16 ft. extension ladder. The extension ladder did not extend three-feet above the upper landing surface b) On the north side of the partially framed residential structure, employees utilized K-Pro 8 ft. A-Frame . The A-Frame ladder was leaned up against the north side of the partially framed residential structure, it was not used according to manufactures guidelines for use.
