Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  On or about April 20, 2022, during the installation of roofing material at a job site located at 1420 N. Main ST, Canton, Ohio 44720; the employer did not ensure employees working on a roof of a commercial facility were protected from a fall of approximately 14.5 feet by utilizing a fall protection system.
