29 CFR  1926.1053(b)(13):The top or top step of a stepladder was used as a step:  a) Jobsite located at - 330 Carson St., Philadelphia, PA, 3rd Floor -Employees used the top step of a self-supporting ladder while in a closed position to access the upper areas of the wall near the edge of the 3rd floor and were exposed to falls of approximately 22 feet.   b) Jobsite located at - 330 Carson St., Philadelphia, PA - 2nd Floor - Employees used the top step of a self-supporting ladder to access the wood platform mounted on the wall of the 2nd floor and were exposed to falls of approximately 12 feet.
