29 CFR 1904.39(a)(2): The employer failed to report an employee's amputation, as a result of a work-related incident, within (24) twenty-four hours: On January 25, 2022, an employee's right distal finger was amputated when it was caught between a rope and a pulley while attempting to open a pedestal on the hanger floor and the incident was not reported to OSHA until April 12, 2022.
