29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:  Guardrail systems per 29 CFR 1910.28(b)(1)(i)(A); Safety net systems per 29 CFR 1910.28(b)(1)(i)(B); or personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems per 29 CFR 1910.28(b)(1)(i)(C).  North Building and South Building: On or about April 20, 2022, the employer did not ensure that fall protection systems were provided at the unprotected edges of the shed roofs.  Employees installing roofs and performing other tasks at heights from the roofs were exposed to fall hazards up to 11 feet.

29 CFR  1910.133(a)(1):  The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  On or about April 20, 2022, the employer did not ensure that each affected employee in the South Building used appropriate eye or face protection when exposed to eye or face hazards from flying particles and/or when using pneumatic nail guns.

29 CFR  1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:   South Building, Grizzly table saw (S/N 1910517):  On or about April 20, 2022, employees were hand-feeding wood stock for ripping on the equipment without a point-of-operation blade guard.  Employees had their fingers and hands exposed to contact with rotating blade likely to result in serious injury.
