29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. On or about April 21, 2022, at 2665-2671 Witzel Avenue in Oshkosh, Wisconsin, the employer did not provide required fall protection safety training for employees exposed to fall hazards while working at heights greater than six feet. Employees were installing roofing materials on a two-story apartment building with unprotected edges.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. On or about April 21, 2022, at 2665-2671 Witzel Avenue in Oshkosh, Wisconsin, employees were exposed to fall hazards while working on a roof at 20 feet above surface level without utilizing fall protection equipment. The A & Son Roofing Improvements Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1442343, citation number 01, item number 01, and was affirmed as a final order on December 30, 2019, with respect to a workplace located at 2945 Hunters Trail; Portage, WI 53901. The A & Son Roofing Improvements Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1322386, citation number 01, item number 01b, and was affirmed as a final order on July 12, 2018, with respect to a workplace located at N114 W15779 Sylvan Circle; Germantown, WI 53022.
