On or about April 21, 2022, at 2665 Witzel Avenue in Oshkosh, Wisconsin, the employer did not provide required forklift operator training for an employee operating a Genie 1056 Telehandler forklift that was used to lift personnel on a work platform attachment during installation of house wrap paper on a two-story apartment building.

29 CFR 1926.454(a)(2): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards; the training shall include the correct procedures for dealing with electrical hazards and for erecting, maintaining, and disassembling the fall protection systems and falling object protection systems being used. On or about April 21, 2022, at 2665 Witzel Avenue in Oshkosh, Wisconsin, the employer did not provide required scaffold safety training for employees exposed to fall hazards greater than ten feet while installing house wrap paper on a two-story apartment building from a forklift work platform.
