29 CFR  1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :   a) At the facility, in the production area, located at 606 Dothan Road, Bainbridge,  GA  39817 , on or about April 7, 2022, the employer did not provide annual audiograms for an employee who was exposed to a time weighted average (TWA) of 90.2 decibels on the A scale (dBA) or an equivalent dose of 103.7% for a run time of 383 minutes, which exceeded the time weighted average of 85 dBA, thus exposing employees to the noise hazards.  b) At the facility, in the wirebound area, located at 606 Dothan Road, Bainbridge,  GA  39817 , on or about April 7, 2022, the employer did not provide annual audiograms for an employee who was exposed to a time weighted average (TWA) of 92.9 decibels on the A scale (dBA) or an equivalent dose of 150.1% for a run time of 390 minutes, which exceeded the time weighted average of 85 dBA, thus exposing employees to the noise hazards.  c) b) At the facility, in the mill area, located at 606 Dothan Road, Bainbridge,  GA  39817 , on or about April 7, 2022, the employer did not provide annual audiograms for an employee who was exposed to a time weighted average (TWA) of 103.7 decibels on the A scale (dBA) or an equivalent dose of 674.8% for a run time of 429 minutes, which exceeded the time weighted average of 85 dBA, thus exposing employees to the noise hazards.

29 CFR  1910.178(m)(7): Fixed jacks were not used to support a semitrailer during loading or unloading when the trailer is not coupled to a tractor to prevent movement of trucks, trailers, or railroad cars while loading and unloading:   a) At the facility, in the production area, located at 606 Dothan Road, Bainbridge,  GA  39817 , on or about April 7, 2022, the employer exposed employees to crushing and struck-by hazards in that a flatbed trailer was not supported with either fixed jacks are having been coupled to a tractor, while the employee loaded the flatbed with a power industrial truck.
