29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Worksite, Baler room, 290 Ballard Road Middletown, New York: On or about April 13, 2022, an employee was exposed to caught-in hazards in that when clearing a jam under the BaleMaster Hogger/Conveyor 1200 Series M.O. 10212, the employee did not shut down the machine using the procedures established for that machine.
