29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:    At the jobsite, employees performing cornice activities used a orange extension cord that was severely cut and frayed, supplying power to a Dewalt circular saw, exposing employees to electrical hazards.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  At the jobsite, employees operating a Paslode nail gun to perform cornice activities without eye and face protection exposing him to the hazard of being struck-by flying objects.  Manuel Cervantes was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1236077, Citation Number 1, Item number 2, and was affirmed as a final order on July 10, 2017, with respect to a workplace located at 584 Spruce Trail, Forney, Texas 75126.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest system.  At the jobsite, employees performing cornice activities were not provided fall protection and exposed to an unprotected fall hazard of approximately 18 feet to the ground below.  Manuel Cervantes was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1236077, Citation Number 1, Item number 3, and was affirmed as a final order on July 10, 2017, with respect to a workplace located at 584 Spruce Trail, Forney, Texas 75126.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  At the jobsite, employees performing cornice activities were not provided fall protection.  The employer did not provide a training program with hazard recognition and avoidance training where part of the job is to be exposed to fall hazards at a heights over 6 feet to the ground below.
