29 CFR  1910.303(b)(2):  Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  On or about June 9, 2022, employees were exposed to electrical hazards due to electrical equipment not being used in accordance with its listing or labeling:  a.  ADFAST Grommet Machine, Model Eagle III 2.0, Serial Number III192-01-22, located on the dividing wall of the production area was plugged into a red extension cord which was plugged into a relocatable power tap which was plugged into the wall outlet. The relocatable power tap was not being used for its intended purpose.   b.  Commercial Electric fan was plugged into an orange pigtail extension cord which was plugged into a yellow extension cord which was  plugged into a relocatable power tap which was plugged into the wall outlet. The relocatable power tap was not being used for its intended purpose.  c.  Sewing Machine, Model 37U2358, Serial Number U20-579001, was plugged into a yellow extension cord which was plugged into a relocatable power tap which was plugged into the wall outlet. The relocatable power tap was not being used for its intended purpose.

29 CFR  1910.303(g)(1):  Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  On or about June 9, 2022, employees were exposed to electrical hazards due to the lack of sufficient access and working space:  a.  Electrical panels in the Production Area  that supply power to various pieces of equipment and lighting were blocked by cardboard boxes.   b.  Electrical panels in the High Voltage Room that supply power to various pieces of equipment and lights were blocked by pieces of plywood, cardboard boxes and other material.

Sewing Machine, Model 37U2358, Serial Number U20-579001,  was plugged into a yellow extension cord that was plugged into a relocatable power tap which was plugged into the wall outlet.  b.  Sinclair  Hoogs Cutter, Type ST-2000-S, Serial Number  273-103, was plugged into a black extension cord that was fastened to the ceiling, wall, and metal conduit with zip ties which was plugged into the wall outlet.
