29 CFR 1926.451(c)(2)(v):Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied. The employer failed to protect employees from fall hazards. This was most recently documented on or about April 26th, 2022, in Papillion, Nebraska. Multiple employees were observed conducting framing operations and working on a platform that wasn't secured to the forklift.

29 CFR 1926.451(g)(1):Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level. The employer failed to protect employees from fall hazards. This was most recently documented on or about April 26th, 2022, in Papillion, Nebraska. Employees were observed conducting framing operations and they were exposed to fall hazards in excess of 16ft because there were no side rails located on the mobile scaffolding.
