29 CFR 1915.77(c): When employees were working aloft, or elsewhere at elevations more than 5 feet above a solid surface, either scaffolds or a sloping ladder, meeting the requirements of Subpart E- Scaffolds, Ladders, and Other Working Surfaces, were not provided to afford safe footing, or protected by safety belts and lifelines meeting the requirements of 1915.159 and 1915.160:  a)	Norfolk Naval Base Vessel 11MRB041: On or about 04/23/2022, an employee working on the mast of the vessel was not provided with safe footing or a personal fall arrest system, as a result fell approximately 20 feet to the ground.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR  1915.152(b):The employer did not assess its work activity to determine whether there were hazards present, which necessitate the employee's use of personal protective equipment:   a)		Norfolk Naval Base Vessel 11MRB04: On or about 04/23/2022, the employer failed to access the work activity to determine whether there was a hazard present in that an employee was tasked with working a component on the mast and fell twenty feet to the ground.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:   Naval Surface Warfare Center - Dahlgren: On or about 04/23/2022, the employer failed to report an in-patient hospitalization within 24-hours in that an employee was hospitalization on April 23, 2022, at 0945 and was reported to OSHA on April 24, 2022, at 1:04 pm.   No abatement certification or documentation required.
