29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee was protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): a) At the metal plant department, employees walking near an input hole to an electric furnace holding tank were not protected by a cover, a guardrail system, a travel restraint system, or a personal fall arrest system, exposing employees to fall hazards.
