29 CFR 1910.119(d)(3)(i)(B): The employer did not include all equipment that was part of the process on piping and instrument diagrams. At the worksite located at 4525 Waco Road, Columbia, Missouri: Employees were exposed to hazards related to highly hazardous chemicals onsite. The employer did not ensure the Process Safety Information depicted on the Piping and Instrument Diagram was up-to-date and accurate. A)[6] Project Gatway PID Set For Record-PID.04 did not accurately depict the installed instruments in that Flow Control Valve EC1A.08 was not identified in the field. B) [6] Project Gatway PID Set For Record-PID.04 did not accurately depict the installed instruments in that Gate Valve EC1C.02 was not identified in the field.
