29 CFR 1904.4(a): The employer did not record each work-related illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: At Westborough Behavioral Healthcare Hospital, LLC located at 300 Friberg Parkway Westborough, MA 01581: On or about March 22, 2022, the employer did not record one instance, among other instances, of an employee work-related COVID-19 infection on the OSHA 300 log or its equivalent. One work-related COVID-19 infection was not recorded on the OSHA 300 form.? The employee was exposed to COVID-19 on March 22, 2022 tested positive on March 24, 2022, and lost 11 workdays. Westborough Behavioral Healthcare Hospital, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1904.4(a), which was contained in OSHA Inspection number 1527259, citation number 2 , item number 1 and was affirmed as a final order on July 27, 2021, with respect to a workplace located at 300 Friberg Parkway Westborough, MA 01581.
