29 CFR 1910.147(c)(4)(i):Hazardous energy control lockout/tagout procedures were not developed, documented, and implemented for the control of potentially hazardous energy when employees were engaged in activities covered by this section: WORKSHOP/SLITTER BAY: The hazardous energy control lockout/tagout procedures were not developed, documented, and implemented for the employees during servicing and maintenance of equipment, such as (but not limited to) the two blade replacement at the separator shaft, overarm, and the take up coil at the 160 Slitter Loopco. On/or about April 25, 2022, the employee operating the pressure to the overarm/separator shaft while another employee was removing the bolt between the separator shaft and take up coil using a wood block on the 160 Slitter Loopco, resulted in a finger amputation.
