29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) On or about April 19, 2022 the employer instructed his employees to use the 6 foot step ladder to gain access to the roof that is 10'  where the ladder did not extend 3' past the edge of the roof exposing employees to fall hazards.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about April 19, 2022 the employer instructed his employees to work from the top of the framed roof applying roof sheeting without the use of fall protection exposing employees to fall hazards.

29 CFR  1926.102(a)(2):The employer shall ensure that each affected employee uses eye protection that provides side protection when there is a hazard from flying objects. Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable.  (a) On or about April 19, 2022 on top of roof the employer instructed the employees to use the air nail guns without the use of eye protection exposing them to struck by hazards.
