29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: a) On or about April 23, 2024, employees were exposed to fall hazards of approximately 10-feet while working from the 1st floor top plate of the framework of a house without using fall protection. b) On or about April 23, 2024, an employee was exposed to a fall hazard of approximately 22-feet and greater while working from the framework of a house without using fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access: a) On or about April 23, 2024, employees were exposed to a fall hazard of approximately 10-feet when they used an aluminum extension ladder that had not been set three feet above the 1st floor landing.
