29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) 104 Lake Avenue, Ithaca, NY 14850: On or about May 3, 2022, while doing a roofing job, employees were climbing a ladder, on the front of the house, that had side rails that did not extend at least 3 feet above the landing surface of the roof they were accessing. b) 104 Lake Avenue, Ithaca, NY 14850: On or about May 3, 2022, while doing a roofing job, employees were climbing a ladder, on the back of the house, that had side rails that did not extend at least 3 feet above the landing surface of the roof they were accessing.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501. a) 104 Lake Avenue, Ithaca, NY 14850: On or about May 3, 2022, while working on a two story tall, approximately 8/12 pitch, residential roof, with no fall protection, employees were exposed to fall hazards greater than 6 feet. Hale Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1543635, citation number 1, item number 1 and was affirmed as a final order on 10/12/2021, with respect to a workplace located at 6795 Route 21, Almond, New York.
