29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about 05/04/2022, at 2985 Iron Wedge Drine, Boca Raton, Florida, employees were using the lower section of an extension ladder for access and egress from a 7-foot-deep sewer manhole.  The extension ladder did not extend at least 3-feet above the street level exposing the employees to a fall hazard.
