29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about May 4, 2022, employees were inside an excavation greater than 5 feet deep grading sewer pipe without an adequate protective system due to improper benching for the soil type present, and no other protective system present.
