29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a)	On or before May 5, 2022, machine specific, step-by-step procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section, such as but not limited to the Steak Line conveyor STC-5. Employees use a hand-held meat hook to dislodge small pieces of meat between the guard and belt itself at the end of the conveyor without implementing LO/TO procedures. This condition exposed employees to laceration, crushing, pinch point, and amputation hazards.  (b)	On April 29, 2022, machine specific, step-by-step procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section, such as but not limited to the Steak Line conveyor STC-5. An employee used a hand-held meat hook to dislodge small pieces of meat between the guard and belt at the end of the conveyor while it was operating. The employee sustained a serious hand injury when the tool became stuck between the guard and the moving conveyor. This condition exposed employees to laceration, crushing, pinch point, and amputation hazards.  Abatement Note: The procedures shall include but are not limited to the following:  1)	A specific statement of the intended use of the procedure:  2)	Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;  3)	Specific procedural steps for the placement, removal, and transfer of lockout devices or tagout devices and the responsibility for them; and  4)	Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.
