Trench - On or about May 9, 2022, employees were observed working in a trench that was between 5 feet and 3.5 feet deep without a ladder or other safe means of egress as the employees had to make a step of greater than 19 inches to exit or climb over a section of pipe to climb up a slope that was approximately 70% or 9x12 pitch, exposing employees to a fall hazard.
