29 CFR 1915.72(a)(3): Portable ladders were not lashed, blocked, or otherwise secured to prevent their displacement: a) Naval Station Norfolk: On or about 04/27/2022, two employees were observed using a 4' A-frame ladder to access a vessel approximately 7' to the working deck without the ladder being secured to prevent displacement. 29 CFR 1915.72(a)(3): The side rails of ladders used for access to any level extended less than 36 inches above that level: a) Naval Station Norfolk: On or about 04/27/2022, two employees were observed using a 4' A-frame ladder to debark a vessel approximately 7' from the ground without grab bars or ladder side rails that extend at least 3' above the work deck. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1915.73(d):When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2): a) Naval Station Norfolk: On or about 04/27/2022, employees working on a trailered vessel with unguarded edges were not provided with fall protection, as a result, were potentially exposed falls from heights greater than 5 feet. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
