29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: a) AFFCO: On or about May 5, 2022 and at times prior, employees were exposed to caught in hazards while performing confined space entry to conduct maintenance and greasing operations on the Wheelebrator. The procedure for Lock Out Tag Out was not communicated to employees prior to performing this task on the specific piece of equipment.

A device such as a panic bar that locks only from the outside is permitted on exit discharge doors. a) AFFCO: On or about May 5, 2022 and at times prior, employees were exposed to entrapment hazards in the event of a fire due to an exit door at the location being installed backwards with the lock facing outside.
