29 CFR  1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.   33 Area Headquarters (new build) Project:  Employees were performing framing, steel erection, rigging, and welding operations on steel beams and used hand protection (leather welding gloves) with torn fingers tips that had duct tape placed over the torn holes.  The employer did not ensure that employees maintained and replaced personal protective equipment when torn. Employees were exposed to dermal burns.

29 CFR  1910.134(h)(1)(i): Respirators issued for the exclusive use of an employee were not cleaned and disinfected as often as necessary to be maintained in a sanitary condition:  (Construction Reference 1926.103)  33 Area Headquarters (new build) Project:  Employees used Miller LPR-100 half mask tight fitting respirators during welding operations which were covered in dust and dirt.  The employer did not ensure respirators were cleaned frequently to maintain them in a sanitary condition.  Employees exposed to upper respiratory irritants.
