29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about May 13, 2022, at the above addressed jobsite, employees using pneumatic nail guns did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.   The Jose Luise Zaruma dba Z 1 Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 152487, citation number 1, item number 1and was affirmed as a final order on June 6, 2021, with respect to a workplace located at 621-633 LeParc Ct., Buffalo Grove, IL 60089.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a)    a. On or about May 13, 2022, at the above addressed jobsite, employees performing residential roofing operations, nine (9') feet three (3") inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.  b. On or about May 13, 2022, at the above addressed jobsite, an employee performing residential roofing operations eighteen (18) feet eight(8) inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.  The Jose Luise Zaruma dba Z 1 Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 152487, citation number 1, item number 2 and was affirmed as a final order on June 6, 2021, with respect to a workplace located at 621-633 LeParc Ct., Buffalo Grove, IL 60089.
