29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems: a) On or about May 13, 2022, employees were exposed to a fall hazard greater than 15 feet, while installation insulation on the roof of a steel erection, without fall protection. b) On or about May 17, 2022, employees were exposed to a fall hazard greater than 15 feet, while installation insulation on the roof of a steel erection, without fall protection. Cristino Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1380683, citation number 1, item number 2, and was affirmed as a final order on July 18, 2019, with respect to a workplace located at 6912 Camp Bullis Rd, San Antonio, TX 78256.

29 CFR 1926.761(b): The employer did not institute a training program and ensure employee participation in the program for each employee exposed to a fall hazard: On or about May 13, 2022, employees were exposed to a fall hazard while installing a roof on a steel erection building, without effective training to recognize and avoid the hazards. b) On or about May 17, 2022, employees were exposed to a fall hazard while installing a roof on a steel erection building, without effective training to recognize and avoid the hazards.
