29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section: a. On or about May 13, 2022, at the facility, employees were changing-out rotary die cutting boards on a rotary die cutter machine, during which time the energy control procedure (Intermediate Energy State/ IES procedure) did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy; therefore, exposing the employees to amputation hazards. b. On or about May 13, 2022, at the facility, employees were conducting weekly Preventative Maintenance/ PM, such as weekly cleaning of the rotary die cutter machine, during which time the energy control procedure (Zero Energy State/ ZES procedure) did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy; therefore, exposing the employees to amputation hazards.
