(a)   On or about 05/13/22,   at the former "Bar 145" undergoing renovations, located at 71 Celebration Drive - College Town in Rochester, New York,  the employer did not enforce the use of a hard hat while employees were installing electrical service above ceiling panels and exposed to overhead hazards such as but not limited to cuts lacerations from T Bar grids or unexpected loose materials.  Employees were aloud to work above suspended ceilings without head protection.
