On or about May 18, 2022 and at times prior, the employer did not ensure that flexible power cords being used to power equipment such as but not limited to portable power tools, monitors, heaters/fans and radios in the facility were used in accordance with instructions included in the listing or labeling. Flexible power cords were plugged in series with other flexible cords providing power to the cords and equipment being used. The following condition exposed employees to electrical shock hazards.

29 CFR 1910.304(g)(5):Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective. a. On or about May 18, 2022 and at times prior, located at the Angle Master Machine, flexible power cords were used to supply power to equipment such as but not limited other flexible power cords, portable hand grinders, fans, and radios. A two-phase flexible power cord was plugged into a three-phase flexible power cord, the following condition failed to provide a permanent, continuous and effective grounding path, thereby exposing employees to electrical shock hazards.
