On or about July 10, 2024, in the production area, the employer failed to ensure that walking-working surfaces were maintained free of hazards due to water from leaking extruders onto the floor, thereby exposing employees to slip, trip, and fall hazards.

29 CFR 1910.212(a): Machine guarding - 29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a. On or about July 10, 2024, the employer did not provide employees operating the Matthews International Corporation, Print-A-Mark Hot Print Brander (Model: 4015, S/N: 1014) with adequate guarding at the point of operation, thereby exposing employees to a caught-in hazard. b. On or about July 10, 2024, the employer did not provide employees operating the Matthews International Corporation, Print-A-Mark Hot Print Brander (Model: 4015, S/N: 1008) with adequate guarding at the point of operation, thereby exposing employees to a caught-in hazard. c. On or about July 10, 2024, the employer did not provide employees operating the Matthews International Corporation, Print-A-Mark Hot Print Brander (Model: 4015, S/N: 1029) with adequate guarding at the point of operation, thereby exposing employees to a caught-in hazard.
