29 CFR 1926.502(d)(15)(I): Anchorages used for attachment of personal fall arrest equipment were not designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two: a) Jobsite located at 2600 Girard Ave, Philadelphia, PA - The employer did not ensure anchorages used for the attachment of personal fall arrest equipment were designed and installed as part of a complete personal fall arrest system, in that the employee wearing a personal fall arrest harness was utilizing another employee as an anchor point, thereby exposing both employees to a fall hazard of approximately 45 feet above the ground. Condition observed on or about May 18, 2022

29 CFR 1926.1053(a)(7): Ladders not specifically designed for such use were tied or fastened together to provide longer sections: a) Jobsite located at 2600 Girard Ave, Philadelphia, PA - The employer did not prevent employees were performing caulking activities to use three ladder sections tied together to access the exterior at the fifth floor of a five story building, thereby exposing employees to falls approximately 45 feet above ground level. Condition observed on or about May 18, 2022.
