29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Building 507 at Hill Air Force Base, UT 84056: On or about May 18, 2022, and at times prior, employees did not utilize procedures for controlling hazardous energy when engaged in minor servicing and maintenance activities for the Monarch VMC RTB in the ID Grind department of the Landing Gear Division. Employees were not required to conduct lockout/tagout or implement effective protection before performing wheel adjustments, which required full bodily entry into the machine. This condition exposed employees to unexpected equipment movement, which could result in crushing and amputation of fingers and hands. (b) Building 507 at Hill Air Force Base, UT 84056: On or about May 18, 2022, and at times prior, employees did not utilize procedures for controlling hazardous energy when engaged in servicing tasks for the Monarch VMC RTB in the ID Grind department of the Landing Gear Division. Employees were not required to isolate equipment or place lockout devices before changing out the insert cutter, which required full bodily entry into the machine. This condition exposed employees to unexpected equipment movement, which could result in crushing and amputation of fingers and hands.
