29 CFR 1926.451(c)(2)(v):Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: The employer failed to protect employees from fall hazards. This was most recently documented on or about May 23rd, 2022, in Gretna, Nebraska. Multiple employees were observed conducting framing operations on a mobile platform that wasn't secured to the forklift.

This was most recently documented on or about May 23rd, 2022, in Gretna, Nebraska 68028. An employee was observed using an "A" frame ladder that was leaning up against the residential structure. The ladder was not being used according to the manufacturers requirement.
