The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): The employer failed to protect employee(s) engaged in powered industrial truck operations from crushing and struck-by hazards. This was most recently documented on May 25th, 2022, in Elkhorn, Nebraska. The forklift operator had not completed forklift training to include a classroom portion, and written test that meets the requirements of OSHA standard 1910.178(l) powered industrial trucks, operator training.
