On or about May 23, 2022, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a)   a. On or about May 23, 2022, at the above addressed jobsite, employees performing residential roofing operations, Twelve (12') feet six (6") inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.  b. On or about May 23, 2022, at the above addressed jobsite, employees performing residential roofing operations, Twelve (12') feet two (2") inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.  Yarovyy Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1357175, citation number 1, item number 1 and was affirmed as a final order on December 6, 2018, with respect to a workplace located at 12827 Blue Spruce Dr., Plainfield, IL 60585.  Yarovyy Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1437674, citation number 2, item number 1 and was affirmed as a final order on July 31, 2020, with respect to a workplace located at 240 Hackberry Dr., Streamwood, IL 60107.
